Traditional Japanese Acupuncture and Moxibustion Concentration Program Agreement: Release and Waiver of Liability
This Waiver and Release of Liability ("Waiver") is executed by the undersigned participant ("Participant") in favor of Golden Ratio Acupuncture Clinic, LLC, its owners, directors, instructors, employees, and agents (collectively referred to as "Organization"). The Participant agrees to the following terms:
1. Program Participation:
The Participant acknowledges that participation in the Traditional Japanese Acupuncture and Moxibustion Concentration Program involves hands-on training, including acupuncture needling and moxibustion, as well as theoretical coursework. The Participant agrees to adhere to all guidelines and protocols established by the Organization throughout the duration of the program.
2. Payment of Fees:
The Participant acknowledges that they have received and reviewed the fee schedule for the program. The Participant agrees to pay all registration fees by the specified deadline of August 4, 2025.
3. Cancellation and Refund Policy:
The Participant understands that once payment has been made, all fees are non-refundable. The Organization does not provide refunds for any reason after payment is completed. In the event of program cancellation by the Organization, a full refund will be issued.
4. Cancellation of Program:
The Participant understands that a minimum of 10 registered participants must be met by August 4, 2025. If this minimum requirement is not achieved, the Organization reserves the right to cancel the program. In such a case, all registered participants will be notified, and any payments made will be fully refunded.
5. Non-Transferable Registration:
The registration for the program is non-refundable and non-transferable. Participants are not permitted to transfer their registration or any associated rights to another individual or replace their spot in the program for any reason. Should the Participant be unable to attend the program, they will forfeit their registration and any associated fees.
6. Attendance:
Attendance at all scheduled classes is highly encouraged to ensure a comprehensive understanding of the curriculum. If a Participant misses any session, it is their responsibility to obtain the course materials and make up for missed content. With the exception of personal and family emergencies wherer the participant and the official representitive have made appropriate agreements, participants must remain in the program for its entire duration in order to receive Certification of Completion.
7. Healthcare and Emergencies:
The Participant certifies that they are in good health and have no medical conditions that would prevent them from participating in hands-on acupuncture and moxibustion training. In the event of an emergency, the Participant consents to receive necessary medical treatment and understands that the Organization is not liable for any injury or health issues that may arise during the program.
8. Acknowledgment of Risks:
The Participant understands that acupuncture, moxibustion, and palpatory examinations involve inherent risks, including but not limited to bruising, discomfort, and other health-related issues. The Participant acknowledges these risks and voluntarily participates in all aspects of the program.
9. Release of Liability:
By signing this Waiver, the Participant agrees to release, hold harmless, and indemnify the Organization from any claims, liabilities, or damages incurred by the Participant as a result of their participation in the program, including but not limited to personal injury, property damage, or any other claims arising from the training.
10. Acceptance of Terms:
By signing below, the Participant acknowledges that they have read and fully understand this Waiver and that they willingly agree to its terms and conditions.
